A complete entry application includes the following: (forms may be obtained online at http://cne.ponyclub.org

  • CNE Commitment form* 

  • Two (One with original signatures) Medical Release forms per competitor*

  • One signed Championship Activity Release form per competitor*

  • One signed Championship Competitor Information Form per competitor*

  • One signed Chaperone Duties form per competitor

  • One signed Coaching form (riding disciplines only)

  • One signed volunteer form*

  • Qualifying Affidavit from (Eventing Only)

  • Two copies of current negative Coggins, current year, correct name (riding disciplines only)

  • One copy of Health Certificate (for shipping)

Volunteers are always needed.  Although not mandatory-, we encourage you to complete the Volunteer form (also found at www.ponyclub.org)
